El trasplantament pulmonar (TP) és una opció de tractament ben consolidada en els pacients amb malaltia respiratòria crònica (ERC). Els pacients amb ERC en estat avançat presenten una intolerància a l'esforç degut tant a la dispnea com a la debilitat muscular, la qual cosa afecta la seva qualitat de vida i a la participació i integració en la societat. És conegut que els pacients després de rebre el TP presenten una millor situació clínica, millorant la funció pulmonar i la seva qualitat de vida, però perdura una disminució de la capacitat d'exercici, però en molts casos també persisteix la disfunció de la massa i força muscular esquelètica i respiratòria. Es coneix que això és degut a factors que afecten tant a l'estructura com a la funció de la fibra muscular provocant una disminució de la capacitat oxidativa del múscul esquelètic, alguns factors relacionats amb l'ingrés perllongat a la Unitat de Cures Intensives (UCI), factors relacionats amb la medicació immunosupressora, entre altres. Moltes d'aquestes alteracions poden millorar-se amb els programes de rehabilitació.
